Cancellations & Services Rendered

  • Appointments must be cancelled 24 hours prior to scheduled time. Appointments cancelled less then 24 hours for any reason will be billed the entire Allowed Amount that we would receive from the insurance.

  • Dr Collins is NOT a provider of Medicare, Medicaid, Workman's Comp, or any Disability paperwork or claims.

  • Work/School excuses are provided upon request at time of visit.

​

  • Any additional forms/letters provided for school/work completed in office at time of visit will be done without a fee, however detailed letters and forms outside your appt

      time will have a charge.
